Refunds will only be issued for treatment that has been paid for in advance and not started. If a patient's account has a balance due, a refund will only be issued for the amount over the balance due. If you have any questions regarding your bill or balance due on your account, please call our office at 503.829.9731.
As noted in our Account and Financial Policy form, it is our policy that co-payment and/or patient portion is due at time of service. We will present you with a treatment plan for your convenience. Please note that the treatment plan is only an estimation of what your insurance will cover and your actual cost may be more or less. You are responsible for any amount not covered by your insurance.
